I don’t understand why on some sites the router works without problems, while in other cases I can’t make it work.
The screenshot shows a simple router setting for the Index.php page
It all seems correct to me, but if I use the address …/index.php everything ok, while with the address …/index it gives me error :443
Does anyone know why?

Port 443 is used for HTTPS, possibly HTTPS is not configured correctly
.htaccess is for Apache web server, you’re using IIS
I don’t know how to solve, just pointing that out
