I always thought about something like this, but the problem is, where do you place that comment?
Maybe a tooltip ?

Hi Franse,
In everyday use, a small icon subtly appears next to a step to show that it has an observation without cluttering the interface.
If you hover your mouse over it, then the popup appears.
Of course, if you click on the step, you'll see text explaining why it was done that way.
This is very important when it's been a long time since you last worked on that step.
Besides giving tips like "look, if you change this, you'll also have to review another process."
